At Family Health Clinic Malvern, GPs work closely with a multidisciplinary team, including physiotherapists, podiatrists, osteopaths, psychologists, pharmacists, and specialist doctors, to provide coordinated support for your health journey. Our goal is to help you live well with your condition and better manage its impact
Chronic condition management focuses on understanding the condition, agreeing on realistic goals, coordinating appropriate services and reviewing progress over time. Outcomes vary between individuals, and a management plan does not guarantee remission or prevention of complications.
Your GP may act as the central coordinator of your care and may recommend further testing through our pathology services or a comprehensive health assessment to better understand your overall health. For example, a pharmacist’s review of your medications can help avoid unnecessary additions and reduce the risk of polypharmacy.
To support this, your GP may develop a tailored GP Chronic Condition Management Plan. Eligible patients may be referred for Medicare-subsidised allied health services in accordance with current Medicare Benefits Schedule requirements.
